1. I hereby RELEASE, WAIVE, DISCHARGE AND FOREVER HOLD HARMLESS, Heritage Empowered and any of its affiliates, staff or representatives associated with this tour from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss,
damage, or injury, including death, that may be sustained during this tour, or to any property belonging to me, whether caused by third party ac􀆟on, or otherwise, while participating in this tour, or while in, on, or upon any of the premises where the tour or various activities may be conducted. Heritage Empowered will exercise due diligence with respect to the safety and well‐being of all participants.

2. I acknowledge the risks involved and hazards connected to all scheduled activities, including but not limited to travel risks. I hereby elect to voluntarily participate in said activities and I understand any and all travel hazards.

3. I further hereby AGREE TO INDEMNIFY AND HOLD HARMLESS Heritage Empowered from any loss, liability, damage or costs, including court costs and attorney fees that I may incur as a result of participating in all scheduled activities.

4. I understand that Heritage Empowered does not provide medical or health insurance coverages for students or attendees during this travel tour.

5. It is my express intent that this Waiver of Liability and Hold Harmless Agreement shall bind the members of my family(ies).

6. I understand that Heritage Empowered is not responsible for airline travel, travel delays, loss of property and/or cancellations.

7. I am aware that Travel Insurance Op􀆟on is available.

8. I understand that my school chaperone will provide information regarding curfew and after-hours activities. I agree to follow the directions of the school chaperones and/or their designees. Heritage Empowered will facilitate all scheduled ac􀆟vi􀆟es. School chaperones are exclusively responsible for enforcing the curfews and Heritage Empowered is not responsible for monitoring students activities after-hours and is not
responsible for any harm caused by a students failure to comply with the standards.

9. IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT I have read the foregoing Waiver of Liability and Hold Harmless Agreement, understand it and sign voluntarily as my own free act. No oral representations, statements, or inducements, apart from the foregoing written agreement, have been made. I am at least (18) years of age and fully competent. I execute this Release as full, adequate and complete consideration fully intending to be bound by same.

If participant is under 18, Parent/Guardian consents to the minor's participation in the Tour and related ac􀆟vi􀆟es.
Minor will be supervised by school chaperones.

I am the parent or legal guardian of the Participant. I understand the legal consequences of signing this document,
including (a) releasing Heritage Empowered from all liability on my and the Participant’s behalf, (b) promising not to sue on my and the Participant’s behalf, and assuming all risks of the Participant’s participation in this Activity/Tour, including travel to, from, and during the Activity/Tour. I allow Participant to participate in this Activity/Tour. I
understand that I am responsible for the obligations and acts of Participant as described in this document. I agree to be
bound by the terms of this document.

I hereby grant Heritage Empowered the irrevocable right and permission to use photographs and/or video recordings of me on Heritage Empowered and other websites and in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation to me.

I understand and agree that such photographs and/or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of Heritage Empowered.

I hereby release, acquit and forever discharge Heritage Empowered, its current and former trustees, agents, officers and employees of the above-named entities from any and all claims, demands, rights, promises, damages and liabilities arising out of or in connection with the use or distribution of said photographs and/or video recordings, including but not limited to any claims for invasion of privacy, appropriation of likeness or defamation.

I hereby warrant that I am eighteen (18) years old or more and competent to contract in my own name or, if I am less than eighteen years old, that my parent or guardian has signed this release form below. This release is binding on me and my heirs, assigns and personal representatives.
